Integrated Care Pathway
The Align Health Integrated Care Pathway (ICP) is a fully funded ACC service created to support people who have sustained complex injuries to their knee, shoulder or back.

The pathway has been developed to assist those requiring a co-ordinated range of rehabilitation services to support their journey to a full recovery.
The range of services will include specialist assessment, physiotherapy and if required, vocational rehabilitation to assist with returning to work. You will be assigned an ICP Navigator who shall assist with keeping you connected with the interdisciplinary rehabilitation team and ACC.
The goal is to wrap the required services around you and your specific goals and needs. Other forms of healthcare services may be required and the pathway aims to include whānau and other caregivers in a culturally appropriate manner.
